Blue Sky Family Health Team
We are a health care organization that includes a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community.
We focus on chronic disease management, disease prevention and health promotion.
Mandate 
It is the role of the Blue Sky Family Health Team to support the Family Health Organization (FHO) by providing services for the FHO patients.
Mission Statement 
The Blue Sky Family Health Team is a multi-disciplinary team of allied health professionals and support staff, committed to excellence in the provision of quality, patient-focused health care services under the direction of the physicians of the Family Health Team and offered in a caring, coordinated and responsive manner.
Vision Statement 
Working together with physicians and their patients, the Blue Sky Family Health Team will strive to achieve optimal health outcomes in preventative care and disease management.
