We are a health care organization that includes a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community.
We focus on chronic disease management, disease prevention and health promotion.
Mandate It is the role of the Blue Sky Family Health Team to support the Family Health Organization (FHO) by providing services for the FHO patients.
Mission Statement The Blue Sky Family Health Team is a multi-disciplinary team of allied health professionals and support staff, committed to excellence in the provision of quality, patient-focused health care services under the direction of the physicians of the Family Health Team and offered in a caring, coordinated and responsive manner.

Join Our Health Care Team

Blue Sky Family Health Team is currently seeking a full-time Master of Social Work (MSW) to join our interdisciplinary team of healthcare providers. The MSW will work collaboratively with all team members to provide comprehensive primary healthcare in consultation with our family physicians.
Qualifications:
- Master of Social Work (MSW)
- Minimum of 2 years recent post-master’s experience in a clinical setting (preferred)
- Registered with the Ontario College of Social Workers and Social Service Workers
- Previous experience in patient care, particularly in a primary care setting (preferred)
- Experience with Electronic Medical Records (an asset)
- Exceptional oral, written, and presentation skills, including strong interpersonal abilities
- In-depth knowledge of chronic illness care, including both clinical and support care
- In-depth knowledge local health program delivery and community-based services

Bilingualism is considered an asset.
The successful candidate will have expertise and comfort working with complex mental health issues. The primary focus of practice will include individual and couples counselling, psychosocial therapeutic functions, and psychotherapy for anxiety, depression, and trauma. The MSW should be knowledgeable about community resources related to mental health and crisis services to ensure a seamless transition to community resources, if appropriate.
