Diabetes Management

The Diabetes Management Program provides patients with Type 2 diabetes education and tools to manage their diabetes.  Our Diabetes Nurse Educator assists with improving the overall health status of patients with diabetes through treatment, follow-up, and the promotion of effective self-management.

Goals;

• To improve the overall health status and quality of life for patients diagnosed with diabetes through full comprehensive diabetic assessments
• To improve patient self-care and awareness of optimal diabetes care
• To prevent and delay chronic disease related to complications of diabetes

Services;

• Comprehensive diabetic assessments: education and treatment based on clinical practice guidelines to monitor management of diabetes including retinal screening, foot care, and blood work
• Promotion of development of self- management goals
• Appointment also scheduled with dietitian for nutritional counselling and education
