Social Work Services

Mental health is an important part of overall wellness. The Blue Sky Family Health Team has a team of social workers who are here to support patients with mental health related issues.

Social workers help people find constructive solutions to resolve problems that affect their day-to-day lives. People see social workers when they are going through a difficult period in their personal, family and/or work life.
Blue Sky Family Health Team social workers are registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and are subject to legislation under the Social Work and Social Service Work Act (1998) and the associated Code of Ethics.

Social work support in the Blue Sky Family Health Team is for adults, 16 years of age or older.

How do patients access Social Work support?

Referrals are generated through the Blue Sky Family Health Team physicians or nurse practitioners.

What do Blue Sky Family Health Team Social Workers do?

The social workers at the Blue Sky Family Health Team are involved in a variety of programs and offer in-house, short-term counselling services for individuals, couples and families.

Social workers help patients:

  • Identify and understand the source of their current stress or difficulty
  • Develop coping skills and find effective solutions to their problems
  • Find and advocate for needed resources in the community

Outline

The counselling process typically involves setting therapeutic goals, the use of assessment and screening tools, educational materials, and “homework” exercises (eg. journaling).  Some of the therapeutic modalities utilized by our team of social workers include mindfulness, cognitive-behavioural therapy (CBT), brief solution-focused therapy (BSFT), narrative therapy, psychoeducation, and Eye Movement Densensitization & Reprocessing therapy (EMDR).
