Chronic Disease Self Management Program (CDSMP)

The “Living Healthy” CDSMP is a six-week workshop that helps people with chronic health conditions to better manage their symptoms and their daily life. Workshop groups meet once a week for 242 hours, over the six weeks. There are typically 5-16 participants in each workshop. The workshop is led by two leaders who have taken special training. These leaders are volunteers, many of whom have chronic conditions themselves. The workshop teaches skills for living a healthy life including:

• Exercise
• Healthy eating
• Stress management
• Communicating with your health care professional
• Managing medications
• Pain and fatigue management
• Planning and problem solving

The “Living Healthy” workshop is a licensed program developed and researched at Stanford University. It follows a standardized format that is proven to help people learn to better manage the symptoms of chronic conditions. It is considered a best practice in chronic disease self-management and is offered in over 25 countries around the world.
